Carrington Place of Toledo is a Medicare and Medicaid provider of skilled and intermediate care.  We provide 24-hour nursing care and supervision for individuals with various needs and acute or chronic health problems.  Whether your stay with us is long-term and you need assistance with activities of daily living like dressing and bathing, or short-term to have therapies so you can return to your previous living setting, our caring and supportive staff is dedicated to meeting your individual needs.

Whether your health problems are minimal or extensive, we have rehabilitation and nursing services to help you reach your personal goals.  Our team meets with you to set up a comprehensive care plan, detailing your personal plan of goals.  These goals encompass all aspects of life, making your stay with us a comfortable and joyous occasion. 

For residents who move in for a short stay with a goal to return home, our therapy department's experienced and highly qualified staff work side-by-side with the nursing department to do everything possible to achieve your goal.  All of our planning, especially discharge planning, a key component, starts the day you move in. 

Professional Services include: you personal physician, 24-hour nursing services and supervision, professional therapy services including, physical, occupational, and speech therapy, licensed dietician consultation, social services, laboratory services, pharmacist consultation, and podiatrist.  Other services provided include: nutritious meals and snacks, laundry and housekeeping services, beauty/barber shop, local van transportation, daily mail delivery, daily activities, medication monitoring, and wound/treatment management.
